What to Bring to Your Appointment

  • Your insurance card

  • Physician referral forms if required by insurance

  • A list of current prescriptions and/or over-the-counter medications you are taking, including dose and frequency

  • All pertinent previous neurological evaluations and/or records

  • Any appropriate reports or MRI records you may have

New patient? 

Please print and bring all patient forms with you to your first appointment. You can find all forms listed below.

We make every effort to run our practice on time. We would therefore appreciate it if you could plan on arriving 15 minutes before your scheduled appointment time in order to allow for parking time.

Patient Forms

  • Masks are required to enter building and office
  • When arriving for your appointment:
    • Come inside and check in with the front desk
      • You have the option to wait in your vehicle until the doctor is ready to see you
  • New patients bring new patient forms with you when you come inside for your appointment
  • Please only ONE person accompanying you to your appointment

Insurance and Payment Information

The Neuroscience Center of Northern New Jersey is participating with Medicare. We also accept most other insurance plans and provide insurance billing.


We request payment at the time of your appointment for services that are not covered by insurance. Your insurance company may also require you to pay a co-payment at the time of your appointment. When necessary, our staff will work closely with patients who require a payment plan.

If you have any questions regarding which insurance plans we accept or any patient billing concerns, please call us at the phone number below. Questions regarding your coverage and benefits should be directed to your employer or insurance company.

Office Policies 

  • Test Results

    • For results of lab work, CAT scans, MRIs, EEG's, and etc., please wait at least 3 days after the last test before calling for the results.

    • Approximately two weeks is needed before all the spinal tap results are available.

    • Please be sure to call our office if you haven't heard about your test results.

  • Prescriptions

    • All prescriptions written at the time of the visit should be taken to your pharmacy to be filled.

    • Check your prescription before calling our office for a refill to avoid extra phone calls.

    • Routine refill requests will be honored during regular office hours only.

    • Be sure to advise your doctor or staff member when you have a mail-a-way pharmacy and/or require a 3-month supply of medication.

  • Fees

    • Appointment cancellation must be at least 24 hours advance notice, otherwise a charge may be credited to your account.

    • Requests for forms, letters, copies of records, etc. will be quoted a fee for service. Payment is usually requested in advance.

    • Check return fee: $33.00
